Hi
Just got off the phone organising travel cover with insureandgo.com. I declared my kidney tx (>5 years) and Type 2 Diabetes (thanks Tacro) and have paid £224.50 for annual multi trip worldwide cover - that includes £102.52 for the declared medical conditions. Seemed pretty good to me particularly as comparethemarket.com when given all the same data, said it could not give me a quote.
That's all, might be of help if you are struggling through the travel insurance maze.

There's a list of possible insurers here:

It does seem to be getting harder to get insurance.. I've found Freedom insurance reasonable.
If travelling in the EU (while we are) its essential to have your EHIC, which you can get FREE here:

NEVER EVER USE A SITE THAT CHARGES FOR EHIC.. Its FAKE
Happy Holidays:)

Re: Travel Insurance Cost
I just got a gap year policy for South America from Hiscox for £300 - 215 for the standard policy and an £85 extra element for declaring the kidney failure and subsequent transplant. Reckon that's not too bad!
PKD/PLD diagnosed 1994, CKD stage 5 Oct 2007, Living Donor Kidney Transplant Dec 2008, still going strong!
