Anyone experienced worm-like white discharge in urine?

30ish year old male need some help and perhaps this is the right forum to ask if anyone encountered this issue and might recognize what could be the problem. Any input would make me very happy since I am at loss myself. Only known kidney issue was an acute kidney failure (Glomerulonephritis) when 12 years old due to upper respiratory infection, that did heal.

10 days ago stringy worm-like white particles started to appear in urine and they have not disappeared yet. They appear only in the very first catch of urine. The rest of the urine is pretty clear.

They look like this and can be up to 3 centimeters long (Warning: Contain pictures of urine samples):

These thread-like particles come with smaller specks, and they appear even if I void again after just 10-15 minutes. They do not mix and they do not sink. They appear sticky. The fact that they appear only in the very first catch of urine I believe might be an important clue.

When testing the very first catch with urine dipstick it reads about +2 or +3 for Leukocytes, trace RBCs (+/-10 RBCs non-hemolyzed) and sometimes trace protein.

When testing midstream urine sample no Leukocytes are detected, but the same amount of trace RBCs (+/-10 RBCs) and sometimes trace protein are detected.

I have had minor pain in right loin for 3 months with foamy urine that started pretty suddenly (or I just haven't noticed before). Actually most of the time the pain or discomfort in the right loin can only be felt when sleeping on left side. I also find trace RBCs when testing on home dipsticks all day except first morning void, and seldom a small trace of albumin on dipstick.

I have gone to the doctors three times due these first issues:

I first did a CT with contrast to check for kidney stones 2 month ago. None were found. Also the hospital dipstick found no WBC, RBC, albumin, sugar, nitrate in their dipstick test. All blood tests where fine including normal GFR, creatinine, liver etc. Only value off was serum potassium at 3.4.

Second time at hospital I got an ultrasound to check right kidney, liver and gallbladder for suspected gallstones. The ultrasound found nothing abnormal. Hospital dipstick and blood all OK. Nothing abnormal found.

Third time at GP last week just a dipstick that showed negative. GP believed a full urinalysis or culture not needed. When mentioning the particles in first catch urine GP say this is normal and no microscopic evaluation ordered. Atleast managed to get a PSA blood test from the GP and a 24 hour protein that I am still waiting for results.

So, anyone who have encountered urine particles looking like this before? What could they most likely be, and what might be the reason, and how serious are they?

Myself I have no clue. I've been thinking about seminal fluid from a prostate gone haywire, mucus or pus, white blood cells, urethritis, damaged kidney tubules, damaged kidney glomeruli, shredding of urehtra or some kind of cancer. But actually I have no clue. That these particles only appear in the first few centiliters of urine while the rest of the urine is clear I feel might be a clue to what it could be.

Any input would be very much appreciated. I feel no good about this whole ordeal.

Looks like Protein to me but I am not a medic, the foam also points that way.
Looking at the test so far, they are gradually eliminating other possible causes, the only thing to do is keep going back until it is solved.

I have the same worms in my urine. I spent many months tracking and researching, visiting doctors and have found the following:
Doctors are conditioned in their education to not seek out answers to identifying elemts in urine. They need to follow a symptom of irregular health and finding things in urine is not considered a problem.

Using a phase contrast microscope, although a normal biological scope will suffice to identify these white often translucent or opaque worms a symptom of Schistosomiasis. The images available on the web are not very helpful as they are mostly captured using high quality equipment and therefor do not necessarily resemble what we see in the urine. If you use a loupe magnifier snd a flashlight you can make out the details of the female worms which are the largest upto 3-5cm and their various anatomical artifacts that can then be linked to this identification. The thin smaller objects are usually the males. The bits and clumps of white material can be remanant of the inhabitation of the bladder wall but I have not identified that yet. It's amazing how doctors disassociate themselves from even attempting to diagnose this disease that plagues 600 million people in the world but very rare outside of Third World countries. The medication to treat it is widely available for animals and outside of the US in abundance but try to get it here through a doctor or pharmacy and you won't have any luck. There are obviously political and cultural reasons this odd deficiency in the medical system exists. You can find the medication online eBay or veterinarian you take it once and they go away. It's very Interesting the lifecycle of this creature that requires both snails and humans in order to mature and procreate. It is not contagious except from us to snails and then from snails back to us. I'd be interested to know if you agree that this is what you have or not. I would hope your information comes from a correct assessment and not by way of opinion even if professional.

I don't believe these are worms. In schistosomiasis or filariasis you could see obvious, intact worms in the urine.

To me, it looks like mucus, probably from the prostate, or semen. Semen causes a froth on the urine surface, which disappears within a minute or so. Mucus might be from a bladder cancer, maybe.

Protein would cause cloudy and frothy urine and not strings like in the pictures.

Phosphates would cause just cloudy urine without strings.

Pus would suggest an infection, which would be likely associated with cloudy and smelly urine.

I have got exactly the same problem!
Strange Wormlike 3 cm objects only in first stage of urination
Doctors told me that only Schistosoma haematobium eggs can be found in Urine since it lives in small blood vessels - they are probably right, but I think that those we see are Dead Schistosoma haematobium that were eliminated from Blood vessels in Bladder or Prostata. Might be from a tiny Fistula.
I tried to learn how the body gets rid of dead worms that have a life span of many years' even Decades! And did not find an answer...
for weeks I have Many Photos from my first urine every time, after which I Catheterize my bladder resulting in clear urine
So the Worms Origin must be lower from the Bladder, probably in Prostata or Penis urethra

I am now in the process of understanding and trying to solve the problem, (myself being Alternative Healer) Kind of being Coordinator to my excellent doctors... :-)
In contrast to my previews belief I am beginning to think that maybe the worm-like things in my first urine are some kind of discharge, Microbes or Semen from blocked or inflamed ducts in my Prostate (a complex Organ still to be completely understood) that might have chronic inflammation, Prostatitis, not so easy to cure Even in my young age of almost 78 :-).
My Doctor Is treating me with Antibiotics (Oflodex 200MG twice a day)
I met twice the Best experts in Israel on infectious diseases, Dr. Yael Paran the head of Travel Medicine (“Sharal”) in the Suraski medical center in Tel-Aviv, And Prof. Steve Berger, world known Microbiologist & consultant. previous head of the Medical Center labs, and gave then 7 vials containing the worm-like discharge – meeting them again on next Thursday hoping for an answer.
I asked Dr. Paran & Prof. Berger if they heard about how does the body get rid of dead Schitosoma (that can live up to 30 years!) and till now I didn't get any answer... So everything is still open to suggestions.
I heard about Cialis as an help to aid curing Prostatitis and asked my Doctor to prescribe it to me (5 MG per day),
I am now on the third day on Cialis and with the antibiotics for 3 weeks there is reduction and form change in the urinary discharge

i have the same problem with stringy discharge or worm like things during first stage urination, this started after major heart surgery 6 weeks ago aortic valve replacement. after the op i had a catheter in place for a few days. 3 weeks ago it started with stringy blood during first stage urination which samples were sent away then doc asked for another sample to be sent away and then he wanted to see me but did not confirm anything or give any medication, but asked me to wait another 2 weeks and provide another sample which is due next week, the day after seeing the doc the blood in this stringy gel or worm has disappeared but im now still left with this stringy gel or worm during first stage urination. im at my wits end with this, any help please

I am experiencing the same symptoms of white stringy in a cloudy urine. It seems that no doctor can help with this. First, I suspected a chlamydia or trichomoniasis bacteria. Took azithromycin for chlamydia but no change. I tried then Flagyl and tetracycline for trichomoniasis, unfortunately, did not work. This proves that the problem is not bacterial because my doctor also prescribed me some very powerful antibiotic to treat possible prostatitis as all the result was negative. Ultrasound of the lower abdomen shows no issue. Kidney test for protein in urine came back negative, prostate PSA was normal. The urine I took it more than 10 times, always come out negative. Ureteroscopy shows nothing abnormal. My urologist confirmed no cancer. I think he did some type of CTC but I really don't remember. This condition started three years ago and still no doctor can figure this out and search on google did not help. All the doctor I talk to are keep saying that I am fine but I know that I am not. I will really appreciate if someone can help with this.

I'm going through this exact same issue, can anyone please give me answers as I have been to a health practitioner, but they had no idea what to do with the specimen as the person was just a young guy. he basically told me I'm over examining my urine and jumping to conclusions, but I know for a fact that I have never had this worm like discharge ever until a month ago, and it is also in the first stage of urination. please can someone give me some answers.
